The availability form collects basic contact, residence-interest, timing, and communication-preference information so Choices can respond. Do not use it to send medical records, diagnoses, treatment histories, identification documents, or other sensitive information.

Detailed applications

Applicants who continue use the Behave portal for the full application. The Choices website does not replace that system.

Website measurement

This website counts page views and broad actions such as call, text, application, and Sobrina interactions. Reporting uses daily aggregate counts by page, general source type, and device category. It does not record form contents, Sobrina questions, names, email addresses, phone numbers, or full page URLs in the analytics dataset. The browser keeps only a broad source category for the current tab session; no analytics cookie is set. Browser privacy signals that request no tracking are respected. Google Analytics is not active on this website.

Resident feedback

The feedback form is separate from admissions and is not continuously monitored. Call 911 if anyone is in immediate danger; call or text Choices for a time-sensitive house matter.
